The Blues took an early lead through Connor Essam's own-goal and dominated the early stages.
Adam Webster's red card turned the tide, however, with Jay Simpson netting the resultant penalty after the centre-back's dismissal.
Ollie Palmer then gave the hosts the lead on the stroke of half-time, before another player saw red.
Orient skipper Mathieu Baudry was sent off for a headbutt on Michael Doyle, with both sides reduced to ten men.
But a free-kick was headed home moments later to give the London side a two-goal cushion, with Pompey fighting hard for an equaliser.
Gareth Evans pulled one back on 66 minutes but - despite having chances to equalise - fell to just their third Sky Bet League 2 defeat of the season.
